To be eligible for the program, the individual must meet the following criteria: 1) the individual is pregnant or have a child under the age of 12 months 2) the individual must have a current or past history of substance abuse, or is at risk of substance abuse 3) a resident of Tuolumne County. There is no age factor for the program. It is a home visiting program in which the client would be assigned a navigator who would assist them in connecting to resources appropriate to their needs (along with accountability, follow up, and support).
